When I was a young lad, I used to think a smear test was exactly as described. I imagined that first the doctor would unroll some blotting paper onto the examination bench. Then the patient would drop her skimpies, sit on the blotting paper and wriggle around a bit, thus providing a 'smear' for analysis.

It seems like they're being referred to as a 'scrape' now, which by the sounds of it is much more precise.
